Support Coordination

We understand that putting plans in place to achieve your dreams and aspirations can appear daunting, but it is a wonderful opportunity for you to take control and be in the driving seat of your life and future. Where your hopes and dreams are at the centre of the support you receive, not what others think you should do or have.

Our CHOICES Support Coordinators will support you to start thinking in a different way; helping you consider which supports you want to engage and who you want involved in delivering your services. We won’t have all the answers on who you should work with to achieve a good life, because that is different for everyone, but we do have the skills, tools and networks to help you choose what a good life is for YOU.

The CHOICES Support Coordinators will work with you and your selected support network to map out your supports for the duration of your NDIS plan. This can be done at your home or wherever you feel comfortable, where we will support you to discuss and identify your goals and identify the steps needed to help you achieve them.
